BENEFITS OVERVIEW

Comprehensive Medical

S & S Cycle has a self-funded medical plan administered by Midwest Security Administrators. To best serve our employees, we currently have two PPO plans to choose from: Midwest Preferred and Alliance. We offer a very “rich” medical plan, at an affordable cost to the employee, with clinic office calls and wellness benefit/well child visits at just a $10 co-pay to the employee.

Prescription Drugs

Medco Health administers our prescription drug benefits. Retail pharmacy co-pays are $25 for a 34-day supply of brand name drugs and/or $10 for a 34-day supply of generic drugs. In addition, we also offer mail-order drug prescriptions for employees taking maintenance drug(s). A 90-day mail-order supply of brand name drugs is a $50 co-pay and a 90-day mail-order supply of generic drugs is just a $20 co-pay.

Dental Insurance

Delta Dental administers S&S Cycle’s dental plan. Employees covered by the dental plan have their preventative check-ups covered at 100%. Basic restorative work is subject to a deductible ($50 - $150 per calendar year, based on coverage elected) and then covered at 80%. Major restorative work is also subject to a deductible and then covered at 50%. The dental plan includes orthodontia and is covered at 50% with a $1000 lifetime per insured limitation.

Vision Insurance

S&S Cycle offers vision insurance through Vision Insurance Plan of America, Inc. (VIPA). Through VIPA, those insured may receive a free eye exam, a pair of frames (up to a $70 retail value) and a pair of standard clear glass, or plastic, single vision lenses per calendar year through an in-network provider. Contact lenses are also available in lieu of the frame and eyeglass lens benefit.

S&S Cycle Term Life Insurance

S&S Cycle automatically insures all of its “regular” employees with term life insurance equivalent to two times their gross annual earnings. For example, if an employee grossed $30,000 a year, that employee would be insured for $60,000.

S&S Cycle Accidental Death and Dismemberment

S&S Cycle also provides accidental death and dismemberment insurance in an amount equal to the basic term life insurance. In the event of accidental loss of life, limb, etc., this coverage would pay in full or in part depending on the loss incurred.

Additional Life Insurance

Additional life insurance is also available to employees to buy-up through Unimerica. Employees may buy-up to 5 times their gross annual earnings. Spousal and dependent life insurance is also available. The amount of spousal life insurance the employee can purchase may not exceed 50% of the amount of additional life insurance the employee has elected on him/herself. The amount of dependent life insurance that the employee may purchase ranges from $1000 to $10,000.
